Transitional Housing and Supportive Services Program
Clients referred to the program attend a seminar at the Transitional Housing Program site to learn about the guidelines, options and expectations. 

Clients with continued interest attend an orientation class led by Case Managers at the sites. 

Clients receive an orientation binder outlining the program. Review of current binder content, and implementation of revisions to ensure standard material across sites, will be completed annually.

At Uplifting The Community Development Corporation, we help those who are willing to help themselves in every facet of their lives, to reach their optimum level of living. 
Case Management and Client Tracking:
Clients are currently tracked and managed by program staff.

Staff at all sites with case managers will follow progress toward meeting pre-housing goals.

Completion of low-risk psych evaluations will be performed by a LCSW. 

Post-Discharge Follow-up:  
Post-discharge care is scheduled at 2 weeks and 4-6weeks with the Transitional Housing Program Case Manager.

Post-discharge complications should be referred to the treating Transitional Housing Program site.


Long-Term Follow-up:  
Clients are scheduled for visits/classes for follow-up at 3mos, 6mos, and 1 year.
